Register a regular doctor
To get better reimbursements.
How to do it
- 1Choose a doctor: a GP or a specialist, in private practice, a health centre or a hospital, with no geographical restriction.
- 2Look for a doctor near your home or school in the ameli health directory (annuaire santé), and call to ask whether they accept new patients as médecin traitant.
- 3At the appointment, ask them to be your médecin traitant: with your carte Vitale, they send the declaration online straight to your fund.
- 4If you do not have your carte Vitale yet, fill in and sign form S3704 with the doctor, then hand it in or post it to your health insurance fund.
- 5Then check in your ameli account that your médecin traitant is registered.
What you need
- Your carte Vitale (or your ameli certificate of entitlement in the meantime)
- Your social security number
- Form S3704 if the declaration cannot be done online
Tips
- Without a declared médecin traitant, you get reimbursed less: do it at your first appointment.
- You can change your médecin traitant later: just declare the new one the same way.
- If you cannot find a médecin traitant, contact your CPAM: it can point you in the right direction.
